Photo: MANISH PAUDEL</figcaption></figure>Five days after Nepal went into a nationwide COVID-19 lockdown, a bulk of the tourists stranded in the Mt Everest region are finally being evacuated to Kathmandu for flights back home.There were two flights by <a href="http://archive.nepalitimes.com/blogs/kundadixit/2015/10/12/rebranding-tourism-with-remodelled-plane/" target="_blank" rel="noopener">Tara Air 16-seat Viking Twin Otters</a> on Thursday and Friday from Lukla airfield, but the evacuation picked up momentum on Saturday with Nepal Airlines and Summit Air conducting at least ten ferry flights to Kathmandu. <figure><img width="650" height="488" src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/2022110911110/636b7a3a9c7e80680e06474ajpeg.jpg" alt=""><figcaption>Tara Air in Lukla Airport. All Photos: KUNDA DIXIT</figcaption></figure>The Australian, British, French, Swiss and American embassies in Kathmandu have been the most active in rescuing their nationals who were up in the mountains when Nepal suddenly announced a lockdown from 16-31 March – grounding all domestic and international flights and putting public transport off the roads. The curfew is expected to be extended to mid-April.There were an estimated 400 trekkers and climbers in the <a href="https://www.nepalitimes.com/here-now/everest-garbage-caravan/" target="_blank" rel="noopener">Everest region</a>, but trekkers were also stranded in the Annapurna and Manaslu circuits as well as Langtang. There are said to be about 800 tourists still in the Pokhara area, and the Nepal Tourism Board estimated that there were <a href="https://www.nepalitimes.com/latest/10000-tourists-stuck-in-nepal-being-evacuated/" target="_blank" rel="noopener">10,000 tourists in Nepal</a> when the lockdown was announced last Tuesday.The weather was looking iffy in Lukla early morning Saturday, but the ferry flights managed to dodge cloud buildup over Lamjura Pass to conduct the flights, pilots said. Some helicopter charters also flew up the Everest trail to rescue trekkers. <figure><img width="960" height="960" src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/2022110911110/636b7a3c9c7e80680e06474bjpeg.jpg" alt=""><figcaption>Notice posted by Nepal Tourism Board regarding tourists stranded in Pokhara.</figcaption></figure>Yeti Airlines and Buddha Air deployed their ATR72s on Thursday and Friday to bring French, Swiss and other European tourists from the Annapurna area, and more flights are being arranged on Saturday.The British Embassy has asked its nationals in various parts of Nepal to send information for possible rescue. It also arranged an escorted bus that departed Pokhara at 8AM Saturday to bring tourists by road to Kathmandu.The evacuation flights were delayed because of the need to arrange lodgings in Kathmandu, and negotiate ferry flights out to the final destination of the tourists. International airlines have to fly in empty to Kathmandu because of the lockdown.There were also bureaucratic delays caused by coordination between various ministries. The Nepal government has come under criticism for not paying as much attention to Nepalis who are also stranded in the mountains. Trekking guides and porters have also been stuck in Lukla and various other climbing routes.“We have been working with the embassies and various ministries to help the tourists, and are taking all the health precautions to fly them out by allowing special charter flights so that they can be with their families,” said Dhananjay Regmi, CEO of Nepal Tourism Board. <img width="650" height="487" src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/2022110911110/636b7a3e9c7e80680e06474cjpeg.jpg" alt="">Qatar Airways in the only airline still making ferry flights, and carried out 600 mainly French and German tourists from Kathmandu to Doha on Airbus330 aircraft on Thursday and Friday. Another flight arrived on Saturday to take stranded Australians back home via Doha.Australian Ambassador Peter Budd posted on the Facebook public group <a href="https://www.facebook.com/groups/2712912542154556/" target="_blank" rel="noopener">Australians Stranded in Nepal</a> that Australian nationals would have first priority, New Zealanders after that, and he also disclosed that if there were seats available his wife and some embassy staff would also be on the flight.There have been concerns about the high cost of one-way tickets, and Budd posted: ‘We have tried to achieve economies of scale by securing the largest possible aircraft …The reality is that aircraft entering Nepal are not permitted to carry passengers. In all likelihood, once arrived in Sydney, the aircraft will also have to return to Doha empty. This adds to the cost.’Faced with a crisis of medical equipment for its hospitals for a future outbreak, the government is sending a Nepal Airline Airbus 330 to Guangzhou on Saturday to bring the items.

More stranded tourists evacuated from Nepal

As the COVID-19 pandemic sweeps the globe, hospitals have been overwhelmed. <a href="https://www.nepalitimes.com/banner/why-nepal-must-watch-about-coronavirus-but-not-panic/" target="_blank" rel="noopener">Nepal is ahead of the curve, but if the case load is to rise steeply </a>as it has done in Italy or Spain, the government needs to start preparing ICUs, ventilators, and protective gear for hospital staff.At present, nurses, doctors, interns and resident doctors and other medical personnel in Nepal’s hospitals lack personal protective equipment (PPE), and they fear for their own lives while <a href="https://www.nepalitimes.com/editorial/this-is-a-test/" target="_blank" rel="noopener">treating patients with suspected coronavirus</a>.Patients can be carriers and spreaders of the virus without showing any symptoms. Thousands have been visiting hospitals to get tested, have chest x-rays done, and could be unknowingly infecting medical staff.“If we find issues with the patient’s lungs after the chest x-ray, we monitor them closely for infection,” says a doctor at a medical college in Kathmandu. “By the time we confirm whether they have been infected, the virus could infect many others in the hospital treating them, like myself.”<figure><img src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/20221109101156/636b7a029c7e80680e064735jpeg.jpg" alt="" width="714" height="960"/><figcaption>A doctor wearing makeshift protective gear while treating a patient in Karnali hospital.</figcaption></figure>Many doctors and even interns do not have a choice, and treat patients without PPE. “If you don’t go to work, the medical college won’t give a certificate,” the doctor complains.Staff at the Sukraraj Tropical and Infectious Diseases Hospital in Kathmandu which gets most of the referrals of suspected coronavirus cases are most at risk. Sagar Kumar Rajbhandary works 12 hour shifts at the Teku hospital, and says PPE are must-have given the number of patients coming in for checkups every day.“We need at least a 100 PPE units right away, but we have not got it yet,” says Rajbhandry. Pandey says other hospitals in the region have also copied the design.Protective gear are being manufactured at four other sites, including by the Innovation Centre of Mahabir Pun (see box), the Karnali Health Science Institute, the Dharan-based BP Koirala Health Science Institute.Although these Nepal-made PPE are not as high quality as imported ones, they at least offer some protection to medical staff. One imported PPE can cost anywhere up to Rs20,000, but a locally manufactured one costs only Rs250.“We got local tailors to make 50 PPEs on the first day itself, they are cheap but effective, and serve the purpose for now,” said Mangal Rawal of the Karnali Health Science Institute.https://youtu.be/c_57ssQF8Ywhttps://youtu.be/mGo7TuZu61ghttps://youtu.be/EVX4SlY28IUTourism entrepreneur Suman Pandey says if the government only gave him permission, her could get workers with experience in making trekking gear to start assembly line manufacture of PPEs. He has already made two prototypes which could be also worn by ambulance drivers and helicopter pilots transporting infected people.The PPE also needs to be disposed of properly after use in the medical wards, the ICU or even in front of a patient getting tested for COVID-19. One doctor or nurse needs at least eight units of PPE every day.<figure><img src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/20221109101156/636b7a079c7e80680e064737jpeg.jpg" alt="" width="720" height="960"><figcaption> </figcaption></figure>Bhogendra Raj Dotel at the Ministry of Health, is currently working to procure more PPEs, but there is a shortage in the world market. In case, all flights have stopped, so there is no way to bring them here quickly.“It is not possible for donor countries to respond because they are dealing with shortages themselves,” Dotel explains. Suppliers who used to source PPEs from Singapore, Thailand, China and India are not getting new units because they have stopped exports.Nepal’s hospitals also have a chronic lack of oxygen cylinders and ventilators even at the best of times. But even these would not be enough if the infection rate rises.The Nepal office of <a href="http://www.nsi.edu.np/en" target="_blank" rel="noopener">Nick Simons Institute (NSI) </a>flew in 11 portable ventilators from the United States and they arrived on 16 March. NSI is also committed to supply 1,000 locally-made PPE to the government.China has also committed to help with medical supplies including PPE, but Mahendra Shrestha of Department of Health Services said they had not yet arrived.<img src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/20221109101156/636b7a099c7e80680e064738jpeg.jpg" alt="" width="640" height="853"><h2>How Facebook came to the rescue</h2>Ma<a href="https://www.nepalitimes.com/banner/drones-to-the-rescue/" target="_blank" rel="noopener">habir Pun</a>, the Magsaysay Award winning Nepali activist, knew that Nepal’s health system would not be able to function unless nurses, doctors and technicians had protective gear for treatment of patients.Since imports were impossible because of flights being cancelled, Pun decided that his centre would start churning out PPEs. He even brought 2,500 metres of fabric, but could not find a single tailor shop open in Kathmandu Valley.So on 23 March, he <a href="https://www.facebook.com/mahabir.pun">posted a message on his Facebook page</a> asking for tailors. He has nearly 100,000 followers, and the word spread quickly. Soon Pun started getting inundated with phone calls from tailors and garment shops all over the city.<figure><img src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/20221109101156/636b7a0b9c7e80680e064739jpeg.jpg" alt="" width="600" height="400"><figcaption> </figcaption></figure>Garment factories in Balaju and Putali Sadak offered to stitch prototypes, which are now ready. “We will now show them to doctors, and if they approve we will start mass producing protective gear and distribute them to hospitals all over the country,” Pun told Nepali Times.

Protecting those who protect us from the epidemic

Nepal goes into lockdown for a week

Photo: RED CROSS</figcaption></figure>The <a href="https://www.nepalitimes.com/opinion/what-is-novel-about-the-novel-coronavirus/" data-saferedirecturl="https://www.google.com/url?q=https://www.nepalitimes.com/opinion/what-is-novel-about-the-novel-coronavirus/&amp;source=gmail&amp;ust=1585031842556000&amp;usg=AFQjCNER7BK9EfiZTZ7HAcrRW7TMnI6jKg">current COVID-19 pandemic</a> has forced many to Google other historic outbreaks, and the search usually brings them to the 1918 influenza epidemic that killed between 20-40 million people – half of them in South Asia.Just over 100 years ago, as Europe was coming out of a ruinous four-year trench warfare, American soldiers brought the flu to the front lines from where it had spread like wildfire across Europe.Although it is called the ‘Spanish’ Flu, the virus first appeared in North America and crossed the Atlantic on US troopships bound for the front lines. It is called Spanish Flu because the virus was making people sick in the Pyrenees long after it had subsided elsewhere.After the war, British Army soldiers returning to India (some of them could very well have been Gurkhas) brought the virus to Bombay from where it spread along India’s vast railway network. 50 in every 1,000 Indians died of the virus.There were two waves of the influenza epidemic. It was probably transferred from returning migratory waterfowl to humans in Kansas in spring of 1918 before jumping across the Atlantic to Europe. The spread slowed over the summer, but returned with a vengeance in the autumn that year.<figure><img src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/20221109101156/636b79ae9c7e80680e064718jpeg.jpg" alt="" width="2400" height="1816"><figcaption> </figcaption></figure>Although there are many books about the Spanish Flu in Western literature, in South Asia there is no properly archived documentation about the tragic pandemic that took many lives.Even Mahatma Gandhi was stricken by the bug, and bodies were piled high along river banks in India as cremation sites ran out of firewood. Unlike COVID-19, the 1918 influenza virus struck people in the 20-40 age group.While it affected 28% of all Americans, killing 675,000 – of the US soldiers killed in World War I, half died of influenza. But surprisingly, the pandemic had very little effect on China.Many viruses have an animal reservoir, and the Spanish Flu was associated with ducks and pigs as is usual for influenza viruses. The <a href="https://www.cdc.gov/coronavirus/mers/index.html" data-saferedirecturl="https://www.google.com/url?q=https://www.cdc.gov/coronavirus/mers/index.html&amp;source=gmail&amp;ust=1585031842556000&amp;usg=AFQjCNE-MAzB2oWqzXngSV-qeu_vZW781w">MERS coronavirus</a> that affected the Gulf region in 2012 is associated with camels, and the <a href="http://archive.nepalitimes.com/news.php?id=2664" data-saferedirecturl="https://www.google.com/url?q=http://archive.nepalitimes.com/news.php?id%3D2664&amp;source=gmail&amp;ust=1585031842556000&amp;usg=AFQjCNFPL8LEypjMiL-Zq9OumVczJ63a9w">SARS coronavirus of 2003</a> transferred from civet cats. While the COVID-19 probably originated in bats like the other coronaviruses, its host is believed to be <a href="https://www.nepalitimes.com/banner/coronavirus-outbreak-may-curb-wildlife-trafficking/" data-saferedirecturl="https://www.google.com/url?q=https://www.nepalitimes.com/banner/coronavirus-outbreak-may-curb-wildlife-trafficking/&amp;source=gmail&amp;ust=1585031842556000&amp;usg=AFQjCNGdAGDJ3BCYbsOftkHyaOipIJecdw">the pangolin</a>.As the ‘Spanish’ flu clearly showed, there is <a href="https://www.nepalitimes.com/opinion/covid-19-nepal-and-the-hygiene-hypothesis/" data-saferedirecturl="https://www.google.com/url?q=https://www.nepalitimes.com/opinion/covid-19-nepal-and-the-hygiene-hypothesis/&amp;source=gmail&amp;ust=1585031842556000&amp;usg=AFQjCNFhpeCjNw-lYwjui_QOmW3PKMnd3g">no evidence to show that we in South Asia have better immunity</a> towards the novel coronavirus COVID-19. So, what can be done?First thing is to increase tests for COVID-19 in suspected patients even without a travel history to affected areas. The more tests we do, the more we will know about the spread of  the actual disease.Read also: <a href="https://www.nepalitimes.com/editorial/this-is-a-test/" target="_blank" rel="noopener">This is a test</a>, EditorialIt is also crucial to do cross-sectional serological studies in communities to see if the virus has been introduced there. This test (unlike the RNA-based diagnostic test for a patient) can be done on asymptomatic people in the community to check for COVID-19 antibodies in the blood, and could give us an idea about the extent of the exposure to the community.Many of these antibody tests are new, and the Chinese have vast experience with them in treating COVID-19. The Nepali government would be well advised to seek Chinese help with antibody testing so that we are better prepared in the days and months ahead. Without more diagnostic and serological tests, we may continue to have a false sense of security as the virus continues to sneak up on us in South Asia.The silver lining (if there is one) in this unfolding COVID-19 pandemic is the attention that it has attracted to the importance of rapid diagnostic tests for other neglected infectious diseases in South Asia like <a href="https://www.nepalitimes.com/banner/tb/" data-saferedirecturl="https://www.google.com/url?q=https://www.nepalitimes.com/banner/tb/&amp;source=gmail&amp;ust=1585031842556000&amp;usg=AFQjCNEq-c2ak8GqLj2idm1BOUPOOAk46A">tuberculosis</a>, <a href="http://archive.nepalitimes.com/news.php?id=18178#.XnhYpkAza70" data-saferedirecturl="https://www.google.com/url?q=http://archive.nepalitimes.com/news.php?id%3D18178%23.XnhYpkAza70&amp;source=gmail&amp;ust=1585031842556000&amp;usg=AFQjCNHZKfS0Lv_GE4wqJrQaZ6OSfqTgdg">typhoid</a>, typhus, or leptospirosis.The tests need to be accurate, simple, affordable, and quick. Rapid diagnostics will not just help find infected people, but also to decide on specific therapy so that unnecessary antibiotics and other treatment are not administered. Remembering a pandemic 100 years ago

In 2013, while writing <a href="https://archive.nepalitimes.com/article/Nepali-Times-Buzz/Flu,294">a weekly health column</a> in this paper, I explored the potential for <a href="https://www.cdc.gov/coronavirus/mers/index.html" target="_blank" rel="noopener">MERS </a>(Middle Eastern Respiratory Syndrome) to spread among Nepalis working with camels in the Gulf states. Despite all the dire predictions of how that coronavirus could be brought back to Nepal and spread in the population, it is still a mystery why there was no MERS outbreak here. Viruses do have their own predilections.This time, although <a href="https://www.nepalitimes.com/banner/why-nepal-must-watch-about-coronavirus-but-not-panic/" target="_blank" rel="noopener">Nepal officially has had only one person diagnosed</a> with the novel coronavirus (COVID-19) who later recovered, it is utterly foolhardy to think that we may escape this pandemic. In Italy, there were only four cases in 20 February, but three days later it soared to 400 cases. That virulence is what is new about this novel coronavirus.Even if Nepal does not have a surge of cases now, there is every likelihood that we will have to deal with an outbreak next winter. In addition how COVID-19 will interact with a summer dengue outbreak like last year in Kathmandu is anyone’s guess. But widespread use of masks for the general public may only deplete the limited supply of masks where they are actually needed in hospitals. The most important prevention is <a href="https://www.nepalitimes.com/banner/covid-19-revives-nepals-wash-drive/" target="_blank" rel="noopener">washing hands </a>properly with soap and water 5 to 6 times per day and avoid touching the face. Keeping a reasonable distance from one another is also important. The elderly need to be especially careful.One of the most crucially helpful decisions that the Chinese government took in December 2019 after the novel coronavirus struck Wuhan was to rapidly share the molecular structure of the virus (its genomic sequence) with the world. This made it possible for drug manufacturing companies like <a href="https://www.modernatx.com/" target="_blank" rel="noopener">Moderna</a>, a biotechnology company based in the US, to start working on a vaccine.Read also: <a href="https://www.nepalitimes.com/banner/a-shot-in-the-arm-for-vaccines-in-nepal/" target="_blank" rel="noopener">A shot in the arm for vaccines in Nepal</a>, Sonia AwaleThe first human trial against COVID-19 will start later on this month. This is incredibly fast because in earlier times, it would take at least six years before a vaccine was developed. Although an effective vaccine is the best defence against COVID-19, what can be done for prevention in Nepal here and now is more important.First, the good news. It is now well known that about 80% of COVID-19 victims will have mild symptoms not requiring hospital admission. Of the rest, 15% will be severe and 5% may be critical, possibly requiring ventilators in an intensive care unit. Even if only 0.5% of patients from South Asia’s population of 1.8 billion require ventilators, it is easy to imagine the tragic scale of this potential  emergency.The Wuhan data from Chinese doctors also revealed that it was the elderly (mostly male) that took the brunt of the disease and that children were not symptomatic, even though they may be infected by the virus. The findings also showed that about 1% of the patients who suffer from  COVID-19 die.This is the stark difference between COVID-19 and other causes of influenza where the death rate is only 0.1%. That is, COVID-19 kills ten times more people, and they are mostly above 60 years old. Although 1% may sound small, actual numbers will be numbing when large populations are infected.<figure><img src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/20221109101152/636b79249c7e80680e0646e6jpeg.jpg" alt="" width="3600" height="2400"><figcaption> </figcaption></figure>Besides vaccines, there are also drug studies going on to fight COVID-19. One such medicine is called <a href="https://www.kaletra.com/" target="_blank" rel="noopener">Kaletra</a>, commonly used for HIV patients. Many other drug trials (including the antimalarial drug cholroquin) are going on.The incubation period (the time from infection to symptoms) was also studied in Wuhan and forms the basis of the quarantine period of 14 days. The average incubation period is five days but may be up to 12 days. To be on the safe side a two-week quarantine is recommended, after which it is unlikely that the symptoms will manifest.Unlike some western countries, China, Singapore, Taiwan, and South Korea practiced widespread contact tracing to control the disease. If someone was diagnosed with COVID-19, the health professionals took a detailed history of  who their contacts (including family and friends) were, and followed up to see if they developed symptoms. What is novel about the novel coronavirus?

In preparation for a COVID-19 outbreak, the Nepal Army set up a quarantine zone at their headquarters in Tripureswor, Kathmandu. The camp, which has 54 tents that were gifted by the Chinese government, can accommodate 108 patients with 2 in each tent.Isolation rooms, regular check-up rooms and a tap water system has also been established.<figure><img src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/20221109101152/636b78ef9c7e80680e0646d1jpeg.jpg" alt="" width="960" height="640"/><figcaption>Quarantine zone at Nepal Army's headquarters. Qatar is one of the most popular overseas destination for Nepali workers, and because of that the Gulf state’s strict measures to stop the spread of coronavirus has had a direct impact on Nepal’s economy.On 9 March Qatar banned the entry of nationals from 14 countries including Nepal, and at least 2,000 Nepali workers in the country have been quarantined. The restriction has also affected Nepal’s economic activities beyond labour migration.<a href="https://www.nepalitimes.com/banner/greens-from-nepal-in-qatars-desert/" data-saferedirecturl="https://www.google.com/url?q=https://www.nepalitimes.com/banner/greens-from-nepal-in-qatars-desert/&amp;source=gmail&amp;ust=1584429126680000&amp;usg=AFQjCNHn7YwEcZztCHzoi81nGBan7Kbocg">Kalyan Ghimire’s company</a> Al Saman used to export three consignments of agriculture produce by air to Doha every week, but that business has ground to a halt since the ban on passengers and cargo."We had just received an order for 5,000 kg of strawberries from Qatar, and had sourced them from a farm in Kakani, but the order was cancelled,” Ghimire says. All Photos: KALYAN GHIMIRE</figcaption></figure>Ghimire had also just started exporting Nepali bottled water from Rasuwa to Qatar, and sent especially packaged 480 bottles for test marketing in Doha to compete with Evian and other brands. However, the COVID-19 scare has put an end to that experiment as well.Ghimire was part of a 40-member team of government officials, entrepreneurs and farmers taking part in the Agriteq 2020 in Doha from 18-22 March, but the trade fair has been called off.“Such ups and downs are to be expected in any business, and I try not to lose sleep over this as it is beyond my control, but let’s hope we can resume exports,” says Ghimire, who credits Qatar’s ambassador to Nepal Yousuf Bin Mohamed for getting Nepali exporters to diversify their product range.<figure><img src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/20221109101152/636b78de9c7e80680e0646cbjpeg.jpg" alt="" width="1024" height="768"><figcaption> </figcaption></figure>Only Qatar Airways still flies Kathmandu-Doha after Nepal Airlines and Himalaya Airlines both suspended operations between the two capitals after the travel ban. Some 40,000 Nepali individuals who were set to go to Qatar to start employment are stuck in Kathmandu. An unknown number of workers in Nepal on vacation from Qatar are also stranded here, with concerns about their visas or work permits expiring.There are 401 COVID-19 cases in Qatar with four recoveries, and there are supposed to be about 2,000 Nepali workers who have been quarantined. On Wednesday there was a spike of 238 new cases in Qatar was related to quarantined expat workers but details on nationalities have not been revealed.<figure><img src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/20221109101152/636b78e19c7e80680e0646ccjpeg.jpg" alt="" width="1200" height="1600"><figcaption> </figcaption></figure>COVID-19 has not distinguished between nationals and expats, politicians and those at the lower rungs of the ladder or between rich and poor. But workers in camps neither have the luxury of social distancing nor the opportunity to work from home."There is definitely a sense of fear among workers,” says Mahesh Nepal, who has been working in Qatar for five years. “There is increased surveillance in the labour camps, and all workers have to undergo tests before and after work each day. The consequences of the coronavirus pandemic goes beyond health to Nepal’s economy.As the top destination for Nepali workers in 2018/19, Qatar is important for Nepal’s remittance-based economy. Doha is also an important aerial gateway for tourist traffic and the Nepali diaspora, and any further flight curtailments will have consequences for Nepal beyond migration.The Saudi and UAE blockade of Qatar for the past two years has actually helped the country become more self-reliant and bolstered its capacity to deal with the epidemic. In addition, Qatar has announced a $23 billion stimulus package to help the private sector weather the crisis.Says Mahesh Nepal: “The impact of the pandemic on the Qatar economy could have been worse due to external disruptions on supply chains or imports.”

Among the many silver linings of the Corona outbreak, The Ass learnt at the friendly neighbourhood watering hole this week, is that we can now stop worrying about Nepali politics. There are more serious things over which to panic.GONe has been criticised for not doing enough to detect and control the virus, but that is unfair. Let me be the devil’s legal adviser here and note that playing dead is Nepal’s time tested method of solving a problem: pretend it does not exist. If you don’t see it, there is nothing to fear. Out of sight is out of mind. It is really effective, you should try it some time.Still, one cannot be more careful during these perilous times, so here are some simple precautions to stop the virus in its tracks before it violates Nepali air space.Doing the namaste is going viral. The hand gesture is back in vogue as an alternative to the handshake, and even Prince Charles is doing it.But the advice about namaste-ing left and right is misguided, and may actually make the problem worse. What if the virus is in one palm and not in the other? By executing a namaskar you have infected your other uncontaminated hand as well, and helped spread the pandemic.A safer alternative is the traditional Maori haka greeting of showing tongues at each other as a welcome gesture. The president could greet new ambassadors at Shitall Nibas with a tongue-lashing next time she receives accreditation from, let’s say, the Plenipotentiary of São Tomé and Príncipe.Health officials are promoting hand washing as the best antidote to the virus. We know this works brilliantly because the most effective way politicians have devised to deal with any problem is to wash their hands off it.If, like most places in Nepal, there is soap but no water, just rub your fingers for 20 seconds, and utter a powerful Vedic mantra. That should kill all coronaviruses lurking in the immediate vicinity.As the flu season ends and we enter the season of political unrest in Nepal, the government can ban all assembly of five or more people, citing the Covid threat, and stop all pesky demos at the Mandala.Domestic airlines can announce further belt-tightening and stop serving peanuts on board. Next to go will be cotton earplugs, which hungry Dhangadi passengers sometimes eat. And no more barf bags, queasy passengers can open the emergency hatch to throw up.If the situation worsens, airlines will switch to gliders which will use updrafts to gain height and cruise to their destinations. A test flight from Pokhara to Jomsom this week landed safely in Ladakh.Meanwhile, security is beefed up at Kathmandu to apprehend any viruses trying to sneak into Nepal without visas. Masked gunmen are posted at the arrival concourse to apprehend suspicious micro-orgasms. Immigration officials are trained to be hostile and make germs feel unwelcome in Nepal.Customs officials at the metal-free x-ray downstairs will now carry out aggressive antibody searches.

Photo: BIKRAM RAI</figcaption></figure>The <a href="https://www.nepalitimes.com/banner/why-nepal-must-watch-about-coronavirus-but-not-panic/">COVID-19 epidemic</a> is subsiding in China where it originated, and is now erupting in outbreak clusters across the world. First carried by international travellers (mainly new year holiday makers from China in January-February), it spread through public places. Interestingly, pilgrimage sites like Qom in Iran, a little known evangelical sect in Daegu in Korea, churches in northern Italy, and a synagogue New York were cluster epicentres.A virus is a strand of genetic material that can infiltrate cell nuclei, splicing itself into the DNA of the host and commanding the organism to be its vehicle for transmission. Being a mutant, the COVID-19 is one step ahead of genetically engineered vaccines, and is so smart it piggy-backs on the mobility of human beings to go around the world.The virus has figured out that it does not make much sense to make the humans it infects so sick that they cannot travel – defeating the purpose of maximum infection. COVID-19 patients can be carriers even though they do not show any outward signs of being infected. Even if they have a sore throat or a cough, they are not bed-ridden and commute to work or travel on business, spreading the virus along the way.This mode of transmission is also designed for maximum effect. After leaping the animal-human barrier, it took the easiest path for human-to-human infection by entering the respiratory tract through touch or cough droplets. The virus likes the cold, which is why it spread so quickly during the northern hemisphere winter.Now that the epidemic has gone global, nationality profiling at airports is absurd. On Monday, police at Nagdhunga stopped buses and asked if there were any Chinese on board.  Removing visa on arrival for travelers from ‘infected countries’ does not make sense anymore – you either go whole hog and stop all outsider travel altogether like Bhutan has done, or you let everyone in after screening.In fact, for all we know Nepal is already an ‘infected country’ we just have not bothered to diagnose anyone. Nepal’s officials appear to believe in a vaccine called ‘Pashupatinath’, and expect the protector deity to see us through this crisis. Who knows, it may be our lackadaisical attitude towards personal hygiene, sanitation or waste management that has given Nepalis a certain immunity against viral infections.Scientist <a href="https://www.nepalitimes.com/banner/why-nepal-must-watch-about-coronavirus-but-not-panic/">Sameer Mani Dixit at the Central for Molecular Dynamics</a> calls this the ‘hygiene hypothesis’ – by allowing germs to thrive we develop an immunity to viral and bacterial infections just like a vaccine would. Dixit says it was “pure miracle” that the 2012 MERS epidemic did not affect Nepal in a big way.The coronavirus epidemic adds to the burden of older existing infections that afflict  Nepalis, and takes attention away from bigger bacterial killers like <a href="https://www.nepalitimes.com/banner/tb/">TB</a>, <a href="http://archive.nepalitimes.com/news.php?id=12158">typhoid</a> and <a href="https://archive.nepalitimes.com/news.php?id=16988">diarrhoea</a>, vector-borne eukaryote parasites like <a href="https://www.nepalitimes.com/banner/mosquitoes-move-up-mountains-as-the-earth-warms/">malaria</a>, or other insect-borne virus like <a href="https://www.nepalitimes.com/here-now/nepal-is-a-microcosm-of-all-of-the-changes-in-world-health/">dengue</a> and encephalitis. A public health system already overstretched to deal with these chronic epidemics is not at all prepared for yet another virulent onslaught.However, we cannot expect superstition or lack of sanitation to save us. Nepal is now linked to the global economy, and as such this worldwide epidemic has hit two of the mainstays of our economy: migration and tourism, which together account for nearly 40% of the GDP.As <a href="https://www.nepalitimes.com/opinion/labour-mobility/crossborder-virus-and-nepali-migrant-workers/">Upasana Khadka shows in her column</a>, Nepal is paying a price for such an overwhelming reliance on migration. The moratorium on migrant workers flying to Korea and Qatar is a gut punch to Nepal’s economy. Nepali workers in the Gulf, Malaysia and Korea-Japan are already affected by the slowdown in the global economy.And the COVID-19 scare hit Nepal right at the start of the <a href="https://www.nepalitimes.com/latest/lets-call-off-visit-nepal-year-2020/">Visit Nepal 2020</a>, which means the country will not meet the already unrealistic target of bringing 2 million tourists this year with an ill-planned campaign. March arrivals, except Chinese tourists, were still all right, but there have been massive cancellations of treks and expeditions for April. As our report shows, the impact on employment and income for service providers in hospitality and farmers dependent on selling produce to hotels is huge. VNY2020 has already been put off, and the government has taken the welcome step of using the budget to improve tourism infrastructure.Despite fear going more viral than the epidemic itself, there are some positive outcomes from COVID-19. Nepal’s hospitality industry has once more recognised the importance of domestic tourism. China’s recognition of the animal-transmission route may actually help <a href="https://www.nepalitimes.com/banner/coronavirus-outbreak-may-curb-wildlife-trafficking/">save endangered species like pangolins</a> from extinction. Only later, after boarding, did it sink in that he would soon be going home after three weeks alone in his dorm room in Jingzhou in China’s Hubei province.Shrestha was halfway into his one year MBBS internship at the Jingzhou No. 1 People’s Hospital, when news came of the <a href="https://www.nepalitimes.com/banner/why-nepal-must-watch-about-coronavirus-but-not-panic/" target="_blank" rel="noopener">coronavirus outbreak in Wuhan</a>  three hours drive away. Shrestha says that among their Chinese peers it was a matter of great pride for the Nepali students that their government had sent a plane specially to take them home. Many African students and those from other Asian countries are still in lockdown in various cities in Hubei.“It was surreal seeing the big Nepal airlines jet taxiing in to the empty terminal in the snow,” Shrestha says. “Whenever I travelled between Nepal and China, it had always been in a Chinese airline.”Read also: <a href="https://www.nepalitimes.com/editorial/when-panic-goes-viral/" target="_blank" rel="noopener">When panic goes viral</a>, EditorialShrestha had been back to Nepal just twice during his six years in China, and it was finding himself at the epicentre of the epidemic that brought him back home for the third time. After completing his studies at Yangtze University in Jingzhou, he was interning at the Gastroeterology Department at No. 1 People’s Hospital when he first heard about the coronavirus outbreak.Shrestha and other students had their temperatures checked five times during the three hour bus ride to Wuhan airport, where they had a through medical exam one more time before boarding.<figure><img src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/20221109101136/636b756d9c7e80680e0623ebjpeg.jpg" alt="" width="2320" height="3088"><figcaption> </figcaption></figure>After the five hour flight to Kathmandu, Shrestha and his fellow passengers were taken to <a href="https://www.nepalitimes.com/latest/nepalis-from-hubei-quarantined-in-bhaktapur/" target="_blank" rel="noopener">quarantine at a converted NEA training facility at Kharipati</a>  where he shares a room with two other Nepali students.“It was great relief to be back home, and although our movement is restricted we feel much freer here than during lockdown in China,” Shrestha says. “It is like being in a college hostel again, and the food is superb.”The evacuees have been spending time hanging out with friends sunning themselves on the terrace and using the free SIM cards provided by the government. However, there is uncertainty about when his university is going to reopen.Shrestha and other students say they also miss China, which has become a second home to them. Many of them speak Mandarin, which has allowed them to make Chinese friends that they left behind.“I really miss China,” says Shrestha, who is trying to see if it is possible to complete his internship in a hospital in Nepal.<figure><img src="https://superdesk-pro-c.s3.amazonaws.com/sd-nepalitimes/20221109101140/636b75709c7e80680e0623ecjpeg.jpg" alt="" width="1080" height="1440"><figcaption> </figcaption></figure>His experience has strengthened Shrestha’s conviction in his choice of profession. He witnessed firsthand how patients had to be sent back home in the early days of the outbreak despite Chinese health officials’ best efforts, because there simply wasn’t enough space to treat everyone. The evacuees will undergo another round of testing after the 2-week quarantine period is over before being given the all-clear.Shrestha cannot wait to see his family. He points out that since the news from China was mostly focused on the deaths from the virus and not the recoveries, his family had been really concerned. He adds: “The joy I will feel when I see them after the quarantine is over will be very different to the joy I felt when I previously came back home from China.”
